IconPackager problem

I am using Win 7 Ultimate x64 SP1 and Icon Packager v10.02. When I start my PC some of the icons that I have changed to my preference do not appear and they will only come back if I use Settings/Rebuild Icon Cache. Can I get some expert ideas please?

PS. Is there a way to start IconPackager at Windows start up?
