Higher Difficulty = Higher Rewards?

Many games offer multiple difficulty levels...
- Easy / Casual
- Normal
- Hard
- Hardcore (perma-death, wipe-your-save)
Should games reward players for playing the game on a higher difficulty?
In what way should the higher difficulty reward the player?
Is playing on a harder difficulty a reward unto itself?
(Also... Should the difficulty setting be locked in?)
