Strange On-Screen [Player Name] In-Game Display Glitch

There have been at least three (3) games I have played in recently that have had a strange glitch where a players name has shown up on screen in the upper center-right, in huge white letters. Once it was [_]Eros, and a couple times it has been my name that displayed.
On two of these occasions, a loud audible "squelch", along with "chirping" sounds could be heard. These sounds lasted only a second or two at the most.
On all occasions, all of the players in-game reported seeing the displayed names, and at least 2 acknowledged the weird sounds.
Anyone else experienced similar phenomena?
