Provided it's approved.. I've created and uploaded one for you to the LogonStudio Vista area of the site. It's very basic and clean looking. Not everyone likes my style, hopefully it works for you

You should have been notifed by email why it was not accepted.
Normally you would have received a second email from the mod that rejected it stating a specific reason why. It would have come from a different email address than the first email.
Ahhhhhhhhhh ok.. Jafo explained this all to me in detail.. He always calms me down. I understand it now.. just email me as I said above, I'll link ya to it.
